Tuesday: 8am-2pm, 4pm-9pm
Wednesday: Closed
Thursday: 8am-2pm, 4pm-9pm
Friday: 6pm-8pm

Before your first appointment

Please fill out the Welcome Letter (PDF) , Intake Form (PDF) , and Consent to Treat (PDF) forms and bring it with you to your first appointment. You may also want to review the Ten Tips and FAQ information pages to insure an incredible healing experience!
